Your first visit is your opportunity to get to know our practice, see our facility, and meet our office team. During your initial appointment, we’ll collect the relevant health history and background information, and perform a clinical exam to establish a baseline for care.

You can save time and expedite your first visit by downloading and completing the "Patient Intake Form” at home. Simply hand the form to our receptionist at check-in. Also, remember to bring your insurance card and an ID with you to your first appointment.
Whether your first appointment is for an eye exam, a consultation, or a second opinion, we do all we can to make your visit a welcoming, informative, and productive experience. Once we gather the relevant background information, examine your eyes and obtain any diagnostic images, we’ll discuss if treatment is required and explain your best options in care.
